Thesis Survey on Intimacy in LDRs - would love help!

I'm a masters student in industrial design and I'm doing my thesis on intimacy (including sexual) in LDRs. I was wondering if anyone here would be willing to fill out a survey to this end. In particular I'm looking for people to interview. I would be incredibly appreciative if you could take a look at the survey, where you'll find additional details in the description. Because I'm a new user I can't post the link to the survey so I will PM you with it if you comment below. The survey is completely anonymous, and I will only reach out to those who provide contact info in the survey.
