Generally we both plan our trips together. Once I went up and surprised him, so obviously I did all the planning myself, but usually we figure out which days we're going to be together, how we're getting to the other one (bus, train, car, etc.), travel times, etc. together. It makes it more exciting for us because then we both have something to look forward to

I plan what we do when he's here, he plans what we do when I'm there. We're both casual with plans though, we don't make many of those and leave a lot of time for improvisation.
My mother is like the OP, she must have every minute of the trip planned. She's good at getting things done and seeing as many things possible in as little time possible. I'm more relaxed though. And not really good at organising.
We try to buy flight tickets as soon as possible, 2 to 6 months ahead. As we see each other about every two months, we normally have at least one visit booked at any moment. It makes it easier to cope with saying goodbye and handling the distance.

The both of us plan out our trips. Just that we tend to alternate in between traveling from one country to another, just to give the other time to save up money for THEIR next trip. I figured that it works and it's only fair.

We both do the planning but I've been doing most of the traveling. My SO is in school and not working so rather than waiting until she does have money, I've been visiting her more. I'm ok with that. She's really good at planning the activities when we are together though! But this Friday she is coming to Canada for the second time! I can't wait.
